Seasons of Life Hospice

Hospice is a program of palliative and supportive services that provides physical, psychological, social and spiritual care for the persons who are faced with a terminal diagnosis or are at the end stages of a disease process that is no longer responding to treatment. The focus of hospice care is comfort care, not curative care. The hospice care team provides pain and symptom management.

Hospice care neither hastens nor prolongs death, but rather affirms life, providing quality and dignity for the remainder of a person’s life. Hospice care is about living. Hospice care treats the person, not the disease and focuses on the entire family, not just the patient.

The Seasons of Life Hospice team consists of a medical director, registered nurses, hospice aides, social workers, bereavement support and counseling, chaplains, pharmacy support and trained volunteers. The hospice care team provides direct care to the patients with 2-3 visits per week, and on an as needed basis. The care team has “on-call” staff available 24/7 to assist patients and families with care as needed.

Our hospice team provides care anywhere a patient calls “home," in a patient’s home, assisted living, or a long term care setting. We also have a 10-bed Residential Hospice, nestled in a wooded area overlooking a private lake at 9511 W. Pleasant Valley Road in Parma. Residents have a private room with an attached patio, full bathroom with shower, small refrigerator and a television. Patients are provided home-cooked meals and round-the-clock nursing care.

Hospice costs are covered by many insurance programs, and for those over 65 years of age, the Medicare Hospice benefit covers most aspects of hospice care. In many incidences, Medicaid covers Hospice services. The Residential Hospice has a daily room and board charge, which is not covered under most insurance programs.

An admission to hospice requires a doctor’s order and an evaluation by a registered nurse. The evaluation can take place in the hospital, patients home, nursing home or assisted living.

Bereavement services are a provision of Seasons of Life Hospice.

The Bereavement Coordinator assists patients and families with support through their loved ones journey, up to 13 months after their journey’s end. The Bereavement Coordinator works with all members affected by the patient’s death. Each family’s situation is unique and services are provided to meet their individual needs. There are many services offered from one-on-one support meetings, to small grief support groups, monthly mailings and telephone support as needed.

 

Volunteers are essential to the support services offered by Seasons of Life Hospice.

Volunteers offer support to patients and staff of the Hospice team. Support services offered to our patients include, but not limited to, companionship, assistance with meals, errands, homemaking tasks, medication pick-ups and funeral home visitation. Supportive services for the staff include, but not limited to, making telephone calls to families, mailings, filing and assisting the cook with meal preparation and serving food to the patients. Support services to the patients and staff include the distribution of linen to patient rooms, and the removal of extra linen. Spiritual volunteers are another supportive service to the patients and staff, they are trained individuals who offer prayer and support as requested by patient and family.
