The Skilled Nursing Facility Units Philosophy
The Skilled Nursing Facility (SNF) is a 27-bed facility providing short-term (approx.
7 days) interim skilled medical/nursing and rehabilitation care between the hospital and discharge to home or to rehabilitation facilities, community nursing facilities, and assisted living apartments. The SNF is both Medicare certified and JCAHO accredited.
Description of the Program
The SNF consists of 9 private rooms and 9 semi-private rooms, as well as a dining room/family room, and therapy/activity area. The SNF is located within Parma Hospital, enabling patients to have access to all Hospital services.
Patient Population
The majority of patients admitted are recovering from cardiac disease/surgery, pulmonary disease, general surgery,
joint replacement, and systemic infections. Ages range from 19 to
90+ years, with the average age in the mid 80s. Patients are admitted from all areas of the Hospital,
occasionally as well as from other hospitals in the greater Cleveland area.
Staffing
Nurses assume a primary role in helping patients make the transition from the hospital to home or to another care setting. Working within an interdisciplinary team, RNs, LPNs, and PCAs provide nursing care, teaching and activities to help patients regain their independence. Medical care is provided by physicians credentialed by Parma Community General Hospital and led by the Skilled Nursing Facility Medical Director.
